For many patients with varicose veins, EVLT (Endovenous Laser Treatment) may decrease the pressure feeding varicose veins, but it does not directly resolve them. In some cases, enlarged branch varicosities are best treated with removing them by microphlebectomy, also known as ambulatory phlebectomy. Tiny 1-2mm micro incisions are made and the vein is teased out with a small hook. It is then divided and removed. The incisions are so small that stitches are not needed to close them. They heal with excellent cosmetic results with only a butterfly closure.

The procedure is done in the office with some oral relaxant and local anesthesia. It takes about one hour and the recovery is minimally painful. There may be some minor bruising.

Immediately following the microphlebectomy, the enlarged, bulging vein is gone! Microphlebectomy is then followed by ultrasound guided foam sclerotherapy for residual branches that are not big enough for microphlebectomy.

The decision to perform microphlebectomy vs. Ultrasound Guided Foam Sclerotherapy is frequently a judgement that balance patient’s goals and sensitivities. In many cases, the microphlebectomy offers the best cosmetic result.
